Free in-home assessment
Call us, and we will schedule a no-cost, no-obligation meeting in your home to assess your medical needs, evaluate your home for safety, and review your public funding eligibility. We call this a Family Meeting and invite all involved relatives to be present.
Public funding support
We will provide you step-by-step advice on how to access the public funding support that all Alberta residents are potentially eligible for. This can cover up to 75% of care costs, and out-of-pocket costs start as low as $150 per week.
Care Plan and Care Team creation
A HomeFree physician creates a custom care plan incorporating your Alberta Netcare records. We collaborate with your existing MDs to ensure all health issues are expertly managed. A personalized Care Team and schedule is created around your preferences, and your family is kept updated through direct communication with a dedicated Care Manager.
Unmatched Medically-Supervised Homecare™
The clinical care with HomeFree’s MSH™ program is comparable to many inpatient facilities, but provided in the comfort of your home. Your case is reviewed weekly by our physician and nursing team, and most issues are managed at home and on the same day.
